This form is typically completed by the sender and collects information about your pet so that we can assess the risk and advise on the most appropriate transport as well as being a confirmation that you have read, understand and accept our Terms & Conditions.
This form is to be completed prior to the transport of all Brachecephalic pets by Dogmovers except for puppies under 4 months of age in High Risk Breeds or under 6 months of age in all other breeds.
